Review student, parent/guardian and school nurse roles then collaboratively develop plan of care with student and parent/guardian based on state regulations, provider orders and NPS Diabetes Management Plan

Discuss field trip plans with student, parent/guardian and teachers

Develop or update 504 plan

Provide student’s teachers with appropriate medical information, plan of care as well as information regarding symptoms and treatments of hypoglycemia and hyperglycemia

Assess student’s competency in self care throughout school year, update plan of care and competency form as needed

Monitor CGM if shared when present in Health Office

Determine communication guidelines with parent/guardian regarding care and supplies

Support student’s development of competency in self care and self advocacy in school
