your receipts. Each path requires that you complete a Claim Voucher depending on your provider:
1. Mail your Claim Voucher and receipts to Benefit Strategies: PO Box 1300, Manchester, NH 03105-1300
2. Fax your Claim Voucher and receipts to Benefit Strategies: 603-647-4668
